What’s Included?

The services included depend on the plan you choose as a member. We offer a range of Ancillary Services as well as Home and Health Care Services to meet your needs.

Ancillary Services

Your care navigator becomes your ally to offer unwavering support. They serve as a crucial connection to your personal long-term healthcare needs. Prepared to assist you in addressing obstacles that arise, they serve as your advocate, ensuring your needs and desires are prioritized. They will collaborate on your behalf with other providers and experts to seamlessly integrate tailored care, all focused on you.

A Home Safety Visit will be provided to members in the first year of membership and as needed there after. These assessments can assist individuals who wish to remain in their homes as they age by identifying risk factors. These recommendations can assist with the over all well-being of members and promote independence.

If you are unable to drive or instructed by your physician not to drive, we can provide transportation. This transportation will only be provided to and from a scheduled procedure that is a medically necessary outpatient surgery or short stay procedure, which may include, but is not limited to, cataract removal, colonoscopies, chemotherapy treatment, and surgical biopsies.

The program will provide an Emergency Response System when needed. It will provide 24 hour service in the event of an emergency and the dispatch center can send help or a designated individual to provide assistance. GPS monitoring is also available.

This is a service offered to connect our members with individuals or businesses within the community relevant to the services they need. It aims to simplify the process of finding solutions by leveraging our networks and community connections. This is a business to member connection and fees are not covered under your plan.

Member shall have access to common areas of Garden Spot Village that are available for the use and benefit of all residents of the community. Member will have access to scheduled social, recreational, spiritual, educational activities , leisure events and, arts and craft health programs designed to meet the needs of the members.

If you become ill or have a scheduled procedure that will make it difficult for you to prepare nutritious meals your plan covers short-term nutrition support. Your Care Navigator will work with you to determine your specific needs and either arrange for meal delivery or arrange for homecare services to support you in this area.

Home and Health Care Services

Home Care services will be provided to members who need assistance with their activities of daily living. You will receive the support you need in the familiar environment of your home.

Adult day services are available to members through GSV (M-F) or an alternate location in Lancaster County. These services are designed for individuals who need physical or cognitive support that will allow family caregivers to work or attend to other daily responsibilities.

Personal Care services are available to members when an individual needs some assistance with their activities of daily living but does not need 24 hour nursing care.  Members may also utilize personal care as a transition back to their home after they have had a change in their medical condition.

Cognitive Care Services are available to members experiencing memory-related challenges, and safety at home has become a concern. Should these full-time services be needed they will be provided in a 24/hr supportive environment tailored to the unique needs of the individual. Coverage for services is based on the plan option chosen.

Nursing and Rehabilitation services can be provided to individuals recovering from an acute medical event.  The goal is to assist individuals in regaining their health, mobility and independence for a safe return to home. Services can be provided at a Garden Spot Community or a community of your choice.

Live In care can be arranged for those individuals that require 24/hr support but desire to remain in their homes.  This arrangement is where a caregiver resides in the home of the person needing care, helping with daily activities around the clock.
